The Hospice LVN is responsible for maintaining Quality Assurance and Improvement from the office setting though some time in the field will be required. This role is accountable for the monitoring the care and documentation and how the standards of practice are met with patients.

For this role, we prefer that the LVN has knowledge of medical terminology and the and the hospice care philosophy.

  • Helps write POC – with the NP
  • Review of patient’s chart
  • Updates to charts
  • NP reviews and ensures it is appropriate
  • Prepare for survey
  • Medication classification
  • Staff the patients
  • Contact the companies like wound service, podiatry and track with excel book, tracking with Hospice MD, patient will be seen by who and call in and ask for copies of reports – updates to charts

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Duties and Responsibilities

  1. Helps families, patients and community members with navigating hospice questions and care
  2. Responsible for the office and administrative aspects of delivering of hospice care and monitoring of the organization’s quality assessment performance improvement (QAPI) program.
  3. Ensures that there are active processes in place to monitor and measure all activities related to QAPI.
  4. Coordinates activities related to clinical record review for data collection. Complies, trends, and reports quality data in the following areas: patient care, safety, risk management, infection control, outcomes, and customer satisfaction.
  5. Works collaboratively with nursing supervisors to assure documentation is complete and consistent with care and reflects legal requirements.
  6. Member of the Patient Grievance Committee collaborates with management staff in the follow-up of concerns expressed by patients, families, and physicians.
  7. Coordinates, educates, and implements monitoring activities for safety, risk management, and infection control.
  8. Acts as a resource to staff in identifying safety and risk management issues for patients and staff.
  9. Reviews all incident reports for completeness, risk, standard of care determination, and appropriate follow-up. Maintains a log for trending and makes recommendations related to trend identified.
  10. Maintains aggregate records of employee orientation and annual competency requirements as part of the QAPI plan.
  11. Monitor organization compliance with regulatory and accreditations standards.
  12. Acts as a resource to Clinical Supervisors regarding standards, policies, regulations,
  13. Acts as a clinical resource to staff
  14. Maintains updated nursing knowledge and skills.
  15. Assists with patient visits as needed.
  16. Assists with administrative call on a rotational basis and clinical calls as needed.
  17. Maintain positive relationships with referral sources.
  18. Maintain a positive image in the community.
  19. Acts as a representative of the organization at community meetings, civic, and professional organizations as needed.

QUALIFICATIONS AND REQUIREMENTS Successful candidate must meet the following.

  • Licensed Vocational Nurse
  • Excellent understanding and use of Excel, PowerPoint, and other data-base programs.
  • Three (2) to five (5) years clinical nursing experience. An emphasis in home care/hospice nursing is preferred.
  • Has two (2) years’ experience in Quality Assurance preferred
  • Excellent understanding of performance improvement, quality assessment, and utilization management.
  • Is self-directed and able to work with minimal supervision/
  • Has excellent coordination and communication skills.
  • Bilingual is highly desirable but not necessary.
